Instructions for Sprint Mail-In Rebate

  1. When you receive your wireless device in the mail, check for a copy of your packing slip, like this.
  2. Get your phone number from your BlackBerry listed under "My Number"
  3. Fill out all information on back of the rebate form. Make sure you fill out everything. See example.
  4. Suggested: Make a copy for your file.
  5. Mail it to the address printed on the rebate form. Don't forget to include a copy of the packing slip too!
